CONSULT WITH A STATE OF NEW YORK APPROVED DOCTOR

After the doctor approves you, you will receive a “Patient ID.” You’ll use this number and the information provided to you from our office to register with the New York State Department of Health (NYSDOH) and complete your application online with the state. Once logged into the state portal, click on the Health Applications icon, and select the “Medical Marijuana Management System” link. The website will provide you with instructions on how to register a patient or caregiver.



You must prove your identity by entering your 9-digit driver’s license or state ID number (uploading a copy of your ID will slow down the approval process). You will also be able to use this number for proof of New York State residency or you can upload one of the following: Pay stub from within the last two months; Copy of your current lease agreement; Mortgage statement from the last two months; A copy of a utility bill from the last two months; NYC ID Card; Official Social Security correspondence showing your current address; Practitioner/hospital bill from the last two months.
